Seafarer’s Passport Application in Poland
In ports such as Gdynia and Szczecin, you can apply for a seafarer’s passport.
There are two ways to submit the documents for obtaining a seafarer’s passport:
- Traditional submission (in person at the Urzad Morski): If you apply in person, no PESEL (Polish identification number) is required.
- Online submission: You need to submit the application through the ePUAP system. To do so, you need a Polish PESEL and a trusted profile. ePUAP portal. A video tutorial on how to submit an application: Video Instruction.
The cost of the service is 200 PLN, which is approximately 50 USD. Both Polish and Ukrainian safety certificates, verified by the Maritime Administration, are accepted (the port authority will verify them in their electronic system when submitting the documents).
Documents Required for Obtaining the Seafarer’s Passport in Poland:
- Application form, which can be obtained directly from the port authority or downloaded from their website.
- A passport-sized photo (3.5×4.5 cm). A tie and white shirt are not mandatory. Photos in white shirts and ties are not allowed. According to recent information, the photo should not be in white clothing.
- A valid international passport.
- Medical examination certificate: Polish, Ukrainian, Bulgarian, Romanian, or any other European certificates are acceptable. The certificate should be valid for at least 30 days after submission.
- Ukrainian seafarer’s passport (even if expired).
- Basic Safety Training and Instruction certificate (Basic Safety Training And Instruction), including A-VI/1-1, A-VI/1-2, A-VI/1-3, A-VI/1-4. Original Ukrainian certificates with printed verification from MORAD are also accepted.
- PESEL number, if available. If not, simply inform the authorities. If you do not have a rental agreement, you can use your hotel address.
After processing, you will receive a payment receipt, which can be paid at the nearest bank, Żabka store, or directly at the port authority via a Polish bank’s mobile app if you have an account in that bank. In Szczecin, payment can be made on-site. The paid receipt should be sent to the port authority’s email: umgdy@umgdy.gov.pl. In 60 days, you will receive the seafarer’s passport. You can check the status of your passport by calling the number listed on the receipt.

Application Process at the Maritime Authority in Poland: At the maritime office, take a number. After a short wait, you will be asked to provide the original BJS certificate and the printed BJS verification. Then, they will request the original passport and may also ask for the SOS certificate. Ensure you have the medical certificate with you. Your passport and certificate will be photocopied.
